Paying with Insurance
Gallo Birth Services uses Lactation Network to process billing through insurance for lactation services. If you are using your insurance and United Health Care PPO, Aetna or Provider Network of America, please contact Lactation Network FIRST to determine your coverage for lactation visits. Let them know that you are wanting to make any appointment with Dominique Gallo with Gallo Birth Services. For those with Anthem Blue Cross and Blue Shield insurance, please use my booking page. At the fault of Anthem and their billing practices I am only able to provide telehealth at this time. Any co-pays will be billed separately and can be paid through the website, check, or cash. Those that are not covered under the above mentioned insurances and would like a lactation consultation will need to pay for the lactation consultation when making an appointment. A superbill will be given at the end of the consult and you will use this to seek reimbursement for the consultation from your insurance provider. Needing help with payment?
Gallo Birth Services strives to make our services affordable and obtainable. Unless you are paying through insurance, all payment is due at the time the service is being rendered. If you are wanting a service but do not have the full payment, please contact us so payment arrangements can be made. Any payment installments arranged will have an additional 2.5% processing fee.
Payment Options
Gallo Birth Services accepts payments in the form of cash, credit/debit cards (including FSA/HSA cards), checks and payment through he website (square). Doula clients please refer to your signed contract as it pertains to payment. Please ask for a receipt if wanting one for records. All payments are due at the time of the service. All classes should be paid with registration. There will be no refunds on classes or lactation consults. Doula clients please refer to your contract as it pertains to refunds, and all refunds will be less the deposit.
